Guarantee
We stand behind every restoration we make and guarantee its integrity based on the following guidelines.
• Five years for porcelain fused to metal restorations
• Three years for monolithic all-ceramic restorations
• Two years for layered all-ceramic resorations
Aesthetic Designs Laboratory guarantees (1), the workmanship of new fixed restorations and (2), that all new fixed restorations will fit the master die or master model if adequate reduction of the prepared tooth was completed according to the manufacturer’s recommendations. Failure of a temporary crown is not included in the guarantee. Failure due to debonding (adhesive bonding) is not considered to be the result of inferior workmanship. Failure due to accident, neglect, abuse, changes in tissue or bone structure, additional tooth loss, or improper dental hygiene is not covered.
What ADL Covers:
ADL will refund, replace or repair defective restorations. All refunds are limited to the amount of the invoice. ADL reserves the right to determine if guarantee is applicable.
What ADL Does Not Cover:
Cash refund for work completed. Cost for removal or reinsertion. Cost for incidental or consequential damages, including inconvenience, lost chairtime, transportation costs, lost wages, pain and suffering, or loss of profits.
If a defective restoration is returned to us within the above time spans, we will repair it or remake it at no cost. Please, be advised that ADL’s warranty does not cover:
• Restoration failure due to insufficient reduction (material thickness less than optimum according to manufacturer’s recommendations)
• Restoration failure due to improper material selection (e.g. All-ceramics in a patient with edge to edge anterior occlusion)
• Cost incurred for removal or insertion, chair time loss, loss of wages, inconvenience or pain.
• Loss of restoration due to bonding/cementation failure.
• Failure of restoration due to retentive factors related to the preparation.
• Remakes or repairs related to tooth fracture, caries, gingival recession or root canal treatment.
• Price of gold used for the remake restoration unless the defective restoration is sent to us.
• Defective restorations caused by defective impressions the dentist was unwilling to retake upon notification from Aesthetic Designs technical team.
• Post-cementation shade changes to restorations.
Payment Terms
Aesthetic Designs will include an invoice with each case delivered to you. A monthly statement will be sent to you following the last day of the month. Payment is due immediately upon receipt of the monthly statement. Your balance is considered late if payment in full is not received by the 15th of the month. A $45 late fee will be applied to your account if payment is received after the 15th. The customer agrees to pay each delinquent balance with a monthly service charge of 18% per annum (11/2% per month) or the maximum permitted by law. The customer agrees to pay all reasonable attorney fees, collection costs and court costs incurred by ADL and any of its affiliates in enforcing any of these terms and conditions. The customer agrees to these terms and conditions as stated on each printed ADL statement, invoice and work authorization. ADL accepts personal checks, cashier’s checks, VISA and MasterCard.
Automatic credit card payments can be set up to clear your current statement balance on the 1st of each month. Please contact our office with your card information if you desire this option.
